The 1095-C report is crucial for generating your employees' 1095-C forms and preparing the necessary file for submission to the IRS. Employees receive a 1095-C form if their employer qualifies as an Applicable Large Employer, subject to the employer shared responsibility provisions under the Affordable Care Act. This Form 1095-C contains information regarding the health insurance coverage offered to employees by their employer.
To generate a 1095-C report, navigate to the "Reports" module and click "Company Reports"
Before generating the report:
- Establish your report settings:
- Select the Setting [Gear] button, complete with all necessary information, and click save when complete.
Once the settings have been established, you can now work towards generating your report.
- Click on the green [Play] button
- Select your parameters
- Year
- Employee: (Leave blank for all employees)
- Show Form 1094-C (Toggle on/off)
- Show form 1095-C (Toggle on/off)
- Once parameters are selected, click on 'Build Report'
- Once your report is built, you can either 'Download' or 'Create File'
- Create File:
- A zip archive that contains two files, Manifest File and Form Data File, will be generated. These are the files that have to be uploaded to the IRS portal.
